A piriformis injection targets the piriformis muscle — a deep rotator muscle located in the buttock that passes directly over the sciatic nerve. When the piriformis muscle becomes tight, spasms, or hypertrophies, it can compress or irritate the sciatic nerve, producing a clinical syndrome known as piriformis syndrome that mimics lumbar radiculopathy (sciatica) but originates in the muscle rather than the spine.
Piriformis syndrome is a common but frequently overlooked cause of buttock and lower extremity pain. Patients typically present with deep, aching pain in the buttock that may radiate down the back of the thigh, often accompanied by tenderness over the piriformis muscle belly and exacerbation of symptoms with prolonged sitting, squatting, or hip rotation activities. The condition is more common in women than men and is frequently misdiagnosed as lumbar radiculopathy, sacroiliac joint dysfunction, or hip pathology.
At Interventional Pain Consultants, we perform piriformis injections using fluoroscopic or ultrasound guidance to ensure accurate delivery of medication into the muscle belly while avoiding inadvertent injection into the sciatic nerve or surrounding neurovascular structures. The combination of anesthetic and corticosteroid provides both diagnostic confirmation and therapeutic relief, and the addition of botulinum toxin (Botox) may be considered for patients with refractory muscle spasm.
With the patient positioned prone, the physician identifies the piriformis muscle using either fluoroscopic landmarks (with or without contrast-guided needle confirmation) or ultrasound visualization. Under sterile technique, a small-gauge needle is advanced into the piriformis muscle belly. For fluoroscopically guided injections, a small volume of non-ionic contrast medium may be injected to confirm intramuscular spread and rule out intravascular or intraneural injection.
Once accurate needle placement is confirmed, a therapeutic injectate is administered. The standard injectate contains a local anesthetic (such as lidocaine or bupivacaine) to provide immediate pain relief and diagnostic confirmation, and a corticosteroid (such as triamcinolone or methylprednisolone) to reduce inflammation and break the cycle of muscle spasm and nerve irritation. In patients with refractory or severe muscle spasm, botulinum toxin (Botox) may be added to the injectate, producing prolonged muscle relaxation by blocking acetylcholine release at the neuromuscular junction.
The injection procedure takes approximately 10-15 minutes. Patients lie face down on the procedure table, and the skin over the buttock is cleansed with antiseptic solution. Local anesthetic is infiltrated into the skin and subcutaneous tissues to minimize discomfort. Patients typically feel pressure and possibly a transient muscle twitch as the needle enters the piriformis muscle, but the procedure is well tolerated overall.
Following the injection, patients are monitored for 15-30 minutes. The local anesthetic provides immediate numbing of the area, and patients are asked to report whether their typical pain is reproduced or relieved — this diagnostic information helps confirm the piriformis muscle as the pain generator. Patients should not drive themselves home due to potential weakness in the leg from the anesthetic effect.
After the anesthetic wears off (typically 4-8 hours), patients may experience mild soreness at the injection site for 1-2 days. The corticosteroid takes 2-7 days to reach peak effect, and patients typically begin to notice significant improvement in buttock and leg pain within the first week following the injection. For patients receiving botulinum toxin, the muscle relaxant effect begins within 5-7 days and reaches maximum benefit at approximately 2-4 weeks.
Patients are encouraged to engage in gentle stretching of the piriformis muscle and hip rotators starting 2-3 days after the injection. Physical therapy is often recommended to address underlying biomechanical factors, including gait abnormalities, hip muscle weakness, and core instability that may have contributed to the development of piriformis syndrome. A series of 1-3 injections may be recommended depending on the patient's response.
The diagnosis of piriformis syndrome is primarily clinical, based on characteristic symptoms and physical examination findings — tenderness over the piriformis muscle, reproduction of pain with resisted hip external rotation (FAIR test), and a normal lumbar spine MRI. Piriformis injections serve both diagnostic and therapeutic purposes: significant relief following a diagnostic block confirms the piriformis muscle as the primary pain generator. Contraindications include active infection, coagulopathy, and known allergy to injectate components.
