Hyaluronic acid injection, also known as viscosupplementation, is a therapeutic procedure that restores the natural lubricating properties of the synovial fluid in arthritic joints. Hyaluronic acid is a glycosaminoglycan naturally present in healthy synovial fluid that provides viscosity, elasticity, and shock absorption to joints. In osteoarthritis, the concentration and molecular weight of hyaluronic acid in the joint fluid are significantly reduced, compromising the joint's ability to absorb mechanical loads and protect the articular cartilage.
By injecting exogenous hyaluronic acid directly into the joint space, viscosupplementation replenishes the depleted natural lubricant, restoring the viscoelastic properties of the synovial fluid. This improves joint biomechanics, reduces friction between articular surfaces, and provides a cushioning effect that protects the remaining cartilage from further mechanical damage. The treatment is FDA-approved for knee osteoarthritis and is widely used off-label for other joints based on robust clinical evidence.
Viscosupplementation is particularly well-suited for patients with mild to moderate osteoarthritis who have not achieved adequate relief from conservative measures such as physical therapy, activity modification, and oral analgesics, and who wish to delay or avoid joint replacement surgery. Clinical studies have demonstrated that hyaluronic acid injections provide statistically significant improvements in pain, function, and patient global assessment compared to placebo, with benefits persisting for 6-12 months after a treatment course.
Under ultrasound guidance, the physician identifies the optimal approach to the joint space. After sterile skin preparation and local anesthetic infiltration, a needle is advanced into the joint under real-time visualization. The specific hyaluronic acid product is then injected according to the manufacturer's protocol — some products require a single injection, while others require a series of 3 or 5 weekly injections.
Once in the joint, the injected hyaluronic acid binds to the surface of the articular cartilage and interacts with the remaining native synovial fluid. The exogenous hyaluronic acid also has biologic effects beyond mere lubrication — it exerts anti-inflammatory activity by scavenging free radicals, inhibits the production of inflammatory mediators, and stimulates the body's own synthesis of hyaluronic acid by synovial fibroblasts. This dual mechanism — both mechanical restoration of viscoelasticity and biologic modulation of the joint environment — accounts for the durable clinical benefit that extends well beyond the half-life of the injected material itself.
The injection procedure is similar to a corticosteroid joint injection and takes approximately 10-15 minutes depending on the joint. Patients are positioned comfortably, and the skin is cleansed with antiseptic solution. Local anesthetic is administered to minimize discomfort, and the hyaluronic acid is injected slowly into the joint space. Some patients experience a sensation of fullness or pressure during the injection due to the volume of the gel-like material.
Unlike corticosteroid injections, hyaluronic acid does not provide immediate pain relief. The benefit builds gradually over 2-4 weeks as the material integrates with the synovial fluid and the biologic effects take hold. Patients may experience mild post-injection soreness or swelling in the joint for 24-48 hours, which can be managed with ice and over-the-counter analgesics.
Patients should restrict vigorous activity involving the injected joint for the first 48 hours following the injection. Light activity and range-of-motion exercises are encouraged to help distribute the hyaluronic acid throughout the joint space. Most patients can resume their normal activities within 2-3 days, though high-impact activities such as running or jumping should be avoided for at least one week.
The treatment course varies by product. Single-injection formulations (such as GelSyn-3, Monovisc, or Synvisc-One) complete the treatment in one visit, with benefit developing over 2-4 weeks. Multi-injection products (such as Hyalgan or Euflexxa) require weekly injections for 3-5 weeks, with gradual improvement noted after each session. The full benefit of the treatment course is typically achieved within 4-6 weeks of the last injection, and the relief commonly lasts 6-12 months.
Viscosupplementation is most effective in patients with mild to moderate osteoarthritis (Kellgren-Lawrence grades II-III) who maintain some joint space and have not yet developed advanced bone-on-bone changes. Patients with severe osteoarthritis (grade IV) typically experience less benefit from viscosupplementation and may be better served by joint replacement surgery. Contraindications include active joint infection, significant joint effusion, known allergy to avian products (for avian-derived hyaluronates), and skin infection over the injection site.
