Corticosteroid joint injections deliver a potent anti-inflammatory medication directly into an arthritic or inflamed joint, providing rapid and targeted relief from pain, swelling, and stiffness. These injections can be performed in nearly any joint in the body — including the knee, hip, shoulder, wrist, elbow, and ankle — and are among the most commonly performed and effective interventional pain procedures in musculoskeletal medicine.
Joint inflammation is a hallmark of many musculoskeletal conditions. In osteoarthritis, the joint lining (synovium) becomes inflamed as cartilage degrades, producing inflammatory cytokines that irritate nerve endings and cause pain. In inflammatory arthritis such as rheumatoid arthritis or psoriatic arthritis, the immune system directly attacks the synovium. Corticosteroids are powerful anti-inflammatory agents that suppress this inflammatory cascade at the molecular level, reducing the production of prostaglandins, leukotrienes, and other inflammatory mediators.
At Interventional Pain Consultants, all joint injections are performed using ultrasound or fluoroscopic image guidance to ensure precise placement of the medication within the joint capsule. Image guidance significantly improves accuracy compared to landmark-based ("blind") injections, particularly in deeper or more complex joints, and reduces the risk of extra-articular injection, tendon injury, or intravascular placement.
Using ultrasound guidance, the physician identifies the optimal needle entry point and trajectory to access the joint space while avoiding neurovascular structures. After sterile skin preparation and administration of local anesthetic, a fine-gauge needle is advanced into the joint under real-time visualization. Once the needle tip is confirmed to be within the joint capsule, a mixture of corticosteroid (such as triamcinolone acetonide, methylprednisolone acetate, or betamethasone) and local anesthetic is injected.
The local anesthetic provides immediate pain relief, confirming accurate placement as the patient experiences numbness in the joint within minutes. The corticosteroid begins working within 24-72 hours and reaches peak effect within 5-7 days. The steroid works by binding to glucocorticoid receptors within the synovial cells, altering gene transcription to reduce the production of inflammatory proteins and enzymes. This anti-inflammatory effect reduces synovitis, decreases joint effusion, and alleviates pain, allowing improved mobility and function.
The injection procedure itself takes only 5-10 minutes. Patients are positioned comfortably, and the skin over the injection site is cleansed with antiseptic solution. A small amount of local anesthetic may be injected into the skin before the joint injection to minimize discomfort. Patients typically feel pressure but not significant pain during the injection.
After the injection, patients may experience immediate numbness from the anesthetic (lasting 4-8 hours). It is important to avoid overusing the joint during this anesthetic phase, as the absence of pain can lead to overactivity and potential injury. Patients should rest the injected joint for 24-48 hours and apply ice if needed for any post-injection soreness.
Most patients experience significant improvement in joint pain and function within 3-7 days following the injection, and the full therapeutic benefit is typically appreciated within 2 weeks. The duration of relief varies depending on the severity of the underlying condition, the specific joint injected, and the patient's activity level. In general, corticosteroid joint injections provide 3-6 months of symptomatic relief.
While corticosteroid injections are highly effective, they do not halt the progression of degenerative arthritis. They are best used as part of a comprehensive treatment plan that includes physical therapy, activity modification, weight management, and appropriate medical therapy. Most insurance plans allow 3-4 injections per year in the same joint, as more frequent injections can accelerate cartilage degradation and increase the risk of complications.
Corticosteroid joint injections are appropriate for patients with moderate to severe joint pain that has not adequately responded to conservative treatments such as oral anti-inflammatory medications, physical therapy, and activity modification. The procedure is contraindicated in patients with active joint infection (septic arthritis), bacteremia, coagulopathy, or known allergy to corticosteroids or any components of the injectate. Patients with uncontrolled diabetes should be advised that steroid injections may temporarily elevate blood glucose levels for 3-7 days.
