A thoracic epidural steroid injection is a fluoroscopically guided interventional procedure that delivers corticosteroid medication into the epidural space of the mid-thoracic spine to reduce inflammation and pain arising from thoracic nerve root irritation. While less commonly performed than cervical or lumbar epidural injections, this procedure addresses a distinct population of patients with mid-back pain and thoracic radicular symptoms that are frequently underdiagnosed.
The thoracic spine presents unique anatomical considerations including the steep downward angulation of the spinous processes (which necessitates a paramedian or transforaminal approach in many cases), the relatively narrow epidural space, and the proximity of the spinal cord at this level. These factors make thoracic epidural injections technically more demanding and underscore the importance of operator experience and high-quality fluoroscopic imaging.
Under fluoroscopic guidance in the anteroposterior and lateral projections, an epidural needle is directed into the thoracic epidural space using either a paramedian or midline interlaminar approach. Due to the steep caudad angulation of the thoracic spinous processes, a paramedian approach is often preferred to access the epidural space more directly. Contrast medium injection confirms appropriate epidural spread and excludes subdural, intrathecal, or vascular placement before medication delivery.
The thoracic epidural space is evaluated with particular attention to the spread pattern, as segmental or unilateral flow may require needle repositioning to achieve adequate coverage of the targeted levels. A combination of corticosteroid and local anesthetic is then administered in volumes appropriate for the thoracic region.
The procedure is performed on an outpatient basis and typically requires 15-30 minutes. The patient is positioned prone. The operator will use careful fluoroscopic guidance throughout. Patients may feel pressure during needle placement but should not experience sharp or shooting pain. After the procedure, a brief period of monitoring in the recovery area precedes discharge.
Mild soreness at the injection site is expected for 1-2 days post-procedure. Patients may resume normal activities within 24-48 hours, avoiding heavy lifting and strenuous activity for the first few days. The therapeutic effect of the corticosteroid develops over 1-2 weeks, with progressive symptomatic improvement during this interval.
The thoracic region presents the highest technical difficulty among spinal epidural approaches due to the narrow epidural space, steep laminar angle, and the presence of the spinal cord throughout this segment.
