A lumbar transforaminal epidural steroid injection (TFESI) is a precisely targeted interventional pain procedure that delivers corticosteroid medication directly to a specific spinal nerve root as it exits the vertebral canal through the neural foramen. This approach provides a higher concentration of anti-inflammatory medication at the exact site of nerve root pathology, making it the preferred technique for treating unilateral, single-level radiculopathy.
TFESI is indicated for patients with radicular pain syndromes that correlate with a specific nerve root level identified through clinical examination and corroborative imaging findings. The transforaminal approach offers distinct advantages over the interlaminar route for targeting ventral epidural pathology, such as disc herniations that compress the nerve root in the axillary or shoulder region.
Under fluoroscopic guidance in the anteroposterior and oblique projections, a 22- or 25-gauge spinal needle is directed toward the target neural foramen using a subpedicular or infraneural approach. The needle tip is positioned in the "safe zone" — the superior, posterior aspect of the foramen — to avoid the radicular artery and dorsal root ganglion. After negative aspiration, a small volume of non-ionic contrast medium is injected to confirm appropriate epidural spread along the targeted nerve root sleeve and to exclude intravascular injection (which occurs in approximately 6-8% of cases despite negative aspiration).
Once safe needle placement is confirmed, a therapeutic dose of corticosteroid and local anesthetic is administered directly around the inflamed nerve root. The transforaminal route delivers medication to the ventral epidural space — the region where most compressive pathology (disc herniations, foraminal stenosis) is located — which the interlaminar approach reaches less reliably.
This is an outpatient procedure requiring 15-30 minutes. The patient lies prone. During needle positioning, the patient may experience a brief reproduction of their characteristic radicular pain as the needle approaches the nerve root — this paresthesia is a favorable sign confirming proximity to the target nerve. The procedure is performed under local anesthesia with optional conscious sedation for patient comfort.
Patients may notice immediate improvement in leg pain from the local anesthetic component, though this effect is transient, typically resolving within 4-8 hours. The corticosteroid's anti-inflammatory effect begins within 2-7 days, with progressive improvement over the subsequent two weeks. No significant activity restrictions are imposed, though patients are advised to avoid heavy lifting, prolonged sitting, or high-impact activities for 24-48 hours.
TFESI requires advanced technical skill due to the proximity of critical structures including the radicular artery (a potential contributor to spinal cord perfusion), the dorsal root ganglion, and the exiting nerve root. Digital subtraction angiography is routinely employed at IPC to detect intravascular contrast uptake and further enhance procedural safety. This procedure is contraindicated in patients with known contrast allergy, active infection, coagulopathy, or prior spine surgery at the target level with residual hardware that may obscure the fluoroscopic target.
