A cervical epidural steroid injection is a fluoroscopically guided interventional procedure that delivers anti-inflammatory medication into the cervical epidural space to reduce inflammation and edema surrounding the cervical nerve roots and spinal cord. This procedure addresses pain, paresthesias, numbness, and motor weakness that originate from the cervical spine and radiate into the shoulders, arms, and hands.
Cervical epidural injections are particularly valuable for patients with cervical radiculopathy who have not achieved adequate symptom control with conservative measures such as physical therapy, oral anti-inflammatory medications, or activity modification. The procedure may be performed via an interlaminar (midline) or transforaminal approach, with the choice determined by the specific pathology, level, and operator preference.
Using fluoroscopic guidance in the lateral or contralateral oblique view, a blunt-tip epidural needle is carefully advanced into the cervical epidural space using loss-of-resistance technique with either air or saline. The cervical epidural space requires particular precision due to its smaller volume (approximately 2-4 mL) and the proximity of the spinal cord. Contrast medium injection under live fluoroscopy confirms appropriate epidural flow and excludes subdural or intrathecal placement before medication is administered.
The volume of injectate is deliberately limited in the cervical region (typically 1-2 mL total) to minimize the risk of spinal cord compression from excessive fluid in the relatively confined epidural space. The corticosteroid component provides sustained anti-inflammatory effect at the targeted nerve roots.
The procedure takes 15-30 minutes. The patient may be positioned either seated upright with cervical flexion or prone depending on the approach selected. Due to the sensitivity of the cervical region, patients are monitored for any signs of transient neurologic symptoms during and immediately after the injection. Arrangements for a responsible adult to drive the patient home are required.
Mild neck soreness at the injection site is common for 1-2 days following the procedure. Patients should avoid heavy lifting, strenuous neck movements, and prolonged driving for 24-48 hours. The corticosteroid's therapeutic effect typically begins within 3-7 days, with continued improvement over the subsequent 2 weeks.
Cervical epidural injections carry distinct technical considerations compared to lumbar procedures. The smaller epidural space, increased vascularity of the cervical epidural venous plexus, and proximity to the spinal cord necessitate meticulous technique and real-time fluoroscopic monitoring.
