Transcutaneous electrical nerve stimulation (TENS) is a non-invasive, drug-free therapeutic modality that uses low-voltage electrical impulses delivered through adhesive electrodes placed on the skin to reduce pain. TENS units are small, battery-powered devices that patients can use at home, providing a safe and effective adjunct or alternative to pharmacologic pain management for a wide range of chronic and acute pain conditions.
The concept of electrical stimulation for pain relief is not new — the practice dates back to ancient civilizations that used electric fish to treat pain. Modern TENS therapy was developed in the 1960s and 1970s based on the gate control theory of pain, which proposed that non-painful sensory input can "close the gate" to painful signals traveling to the brain. Since then, research has revealed additional mechanisms including activation of descending inhibitory pain pathways and release of endogenous opioids.
TENS therapy is particularly valuable as part of a comprehensive pain management plan because it is entirely patient-controlled, has no systemic side effects, does not interact with medications, and can be used as frequently as needed without concerns about tolerance, dependence, or toxicity. It empowers patients to take an active role in their pain management and can significantly reduce reliance on analgesic medications.

TENS therapy operates through several neurophysiologic mechanisms. The primary mechanism involves activation of large-diameter (A-beta) sensory nerve fibers through electrical stimulation. According to the gate control theory, activation of these fibers inhibits the transmission of pain signals carried by smaller A-delta and C fibers at the level of the spinal cord dorsal horn, effectively "closing the gate" to pain perception. Additionally, TENS activates descending inhibitory pathways from the brainstem that modulate pain processing at the spinal level, and stimulates the release of endogenous opioids (endorphins and enkephalins) that bind to opioid receptors in the central nervous system.
TENS devices offer multiple stimulation modes. Conventional TENS uses high-frequency (50-100 Hz), low-intensity stimulation that produces a comfortable tingling sensation without muscle contraction. This mode is best suited for acute pain and provides relatively rapid-onset but shorter-duration relief. Acupuncture-like TENS uses low-frequency (1-5 Hz), high-intensity stimulation that produces visible muscle twitching. This mode activates descending inhibitory pathways and endogenous opioid release, providing slower-onset but longer-lasting analgesia. Some devices offer burst modulation and other pattern stimulation to reduce neural adaptation and maintain efficacy over extended use.
Electrodes are placed on the skin over or near the painful area, along the nerve pathway supplying the painful region, or at specific dermatomal segments corresponding to the pain source. The physician or therapist instructs the patient on optimal electrode placement based on their specific pain pattern and anatomy.
TENS therapy begins with a fitting and instruction session during which our clinical staff demonstrates proper device operation, electrode placement, and hygiene. Electrodes are applied to clean, dry skin over the treatment area. The device is turned on and the intensity is gradually increased until the patient feels a comfortable tingling or buzzing sensation without muscle contraction. The patient is taught how to adjust settings and how to properly care for electrodes and the device.
A typical TENS session lasts 30-60 minutes and can be repeated 2-4 times daily or used as needed for pain flares. The device can be used during daily activities, allowing patients to remain active while receiving pain relief. There is no recovery time or downtime associated with TENS therapy. Patients may notice that the sensation diminishes slightly over time as neural adaptation occurs — this is normal and can be managed by adjusting settings or taking brief breaks between sessions.
TENS therapy has no recovery period as it is a purely external, non-invasive modality. Patients can use the device immediately and as frequently as needed without restrictions. Some patients experience mild skin irritation from the adhesive electrodes, which can be managed by rotating electrode positions, using hypoallergenic electrodes, or applying skin barrier products. The most common side effect is transient muscle soreness if the intensity is set too high, which resolves when the intensity is reduced.
Patients typically notice the analgesic effect during the first few TENS sessions, though optimal benefit may require some experimentation with electrode placement and device settings to find the most effective configuration for their specific pain pattern. Many patients find that regular, scheduled use provides more consistent relief than using TENS only during pain flares.
TENS therapy is safe for most patients but has important contraindications. It should not be used over the carotid sinus (front of the neck), over the eyes, through the chest in patients with pacemakers or implantable cardioverter-defibrillators (ICDs), over open wounds or infected areas, over areas of impaired sensation, or over the abdomen during pregnancy. TENS should not be used while driving or operating machinery due to the potential for sudden unexpected stimulation or distraction.
