Evidence-based guidelines recommend a multimodal approach combining medications from different classes that target distinct pain pathways through complementary mechanisms of action. How It Works

The medication management process begins with a comprehensive initial evaluation including detailed pain history, prior medication trials and responses, review of medical records, assessment of medical and psychiatric comorbidities, and evaluation of substance use history and risk factors for medication misuse. Our physicians classify the patient's pain by type (nociceptive, neuropathic, nociplastic, or mixed) and mechanism, as this determines the most appropriate pharmacologic targets.

Based on this assessment, a personalized medication plan is developed using a stepwise, multimodal framework. First-line agents may include non-opioid analgesics (acetaminophen, NSAIDs), adjuvant analgesics for neuropathic pain (gabapentinoids, SNRIs such as duloxetine and venlafaxine, tricyclic antidepressants), muscle relaxants, topical agents (lidocaine, diclofenac, capsaicin), and migraine-specific medications as indicated. When appropriate and after careful risk-benefit analysis, opioid therapy may be considered using the lowest effective dose with strict monitoring protocols.

Regular follow-up visits are scheduled to assess treatment response, monitor for adverse effects, adjust dosing, and evaluate the ongoing need for each medication. Treatment plans are dynamic documents that evolve with the patient's clinical status and treatment response. What to Expect

During your initial medication management consultation, expect a thorough and unhurried evaluation lasting 30-60 minutes. You will be asked about your pain characteristics, prior treatments, current medications, allergies, medical history, and any concerns about medication side effects or dependence. Please bring all current medications — including prescriptions, over-the-counter products, and supplements — to your appointment.

Our physicians will explain the rationale for each recommended medication, including expected benefits, potential side effects, and monitoring requirements. You will receive clear instructions about dosing, administration, and what to do if you experience adverse effects. Follow-up visits are typically scheduled every 1-3 months depending on the complexity of the regimen and the need for ongoing monitoring.

Recovery

Medication management is not a procedure with a recovery period but rather an ongoing therapeutic process. When new medications are initiated, patients may experience initial side effects such as drowsiness, dizziness, or gastrointestinal symptoms that typically improve over 1-2 weeks as tolerance develops. Dose adjustments are made gradually to maximize tolerability. Changes in pain levels may take several weeks to become apparent, particularly for neuropathic pain medications that require dose titration to therapeutic levels.

Our team provides ongoing support between visits through our patient portal and telephone communication. Patients are encouraged to report any concerns about medication effectiveness or side effects promptly so that adjustments can be made in a timely manner.
