Genicular nerve radiofrequency ablation (RFA) is a advanced, outpatient, fluoroscopy-guided thermal neuroablative procedure that delivers precisely controlled radiofrequency energy to the articular branches (genicular nerves) that provide sensory innervation to the knee joint capsule. By creating thermal lesions at temperatures of 80-90 degrees Celsius adjacent to these nerves, the procedure disrupts their ability to conduct pain signals from the osteoarthritic or degenerated knee to the central nervous system, producing a prolonged period of clinically meaningful pain relief. Genicular nerve RFA is indicated exclusively for patients who have achieved a positive response — defined as 50% or greater pain reduction — following diagnostic genicular nerve blocks, confirming that their knee pain is mediated by the targeted nerves.
The clinical significance of genicular nerve RFA in contemporary pain management cannot be overstated. Knee osteoarthritis affects approximately 15-20% of adults over the age of 45, and its prevalence continues to rise with the aging population and increasing rates of obesity. Total knee arthroplasty remains the definitive surgical treatment for end-stage OA, but a substantial proportion of patients are not surgical candidates due to medical comorbidities, advanced age, or personal preference, while others experience persistent pain following otherwise successful joint replacement. Genicular nerve RFA offers these patients a minimally invasive, evidence-based, repeatable, and cost-effective alternative that provides durable analgesia — with published studies reporting 60-80% of patients achieving 50% or greater pain reduction lasting 6-12 months — and delays or eliminates the need for joint replacement in carefully selected patients.

Our protocol incorporates three-dimensional anatomic targeting of the superomedial, superolateral, and inferomedial genicular nerves using multiplanar fluoroscopic imaging, sensory and motor stimulation testing to confirm accurate cannula placement and to avoid unwanted motor nerve injury, and controlled thermal lesioning with continuous temperature and impedance monitoring.
The patient is positioned supine on the fluoroscopy table with the knee in neutral extension and a bolster beneath the popliteal fossa. After sterile preparation and draping, the procedure begins with an anteroposterior fluoroscopic view of the knee to identify the three standard target locations: the superomedial genicular nerve at the junction of the femoral shaft and medial femoral condyle (typically at the 10 o'clock position on the medial condyle), the superolateral genicular nerve at the junction of the femoral shaft and lateral femoral condyle (at the 2 o'clock position on the lateral condyle), and the inferomedial genicular nerve at the junction of the tibial shaft and medial tibial plateau.
At each target site, the skin and subcutaneous tissues are anesthetized with 1% lidocaine, and a 22-gauge, 10-cm radiofrequency cannula with a 5-10 mm active tip is advanced under fluoroscopic guidance until the tip contacts the periosteum at the predetermined target coordinate. Precise bony contact is essential — the genicular nerves run directly adjacent to the periosteum, and the thermal lesion must encompass the nerve within the 5-10 mm radius of the active tip. After confirming satisfactory cannula position on both anteroposterior and lateral fluoroscopic views, sensory stimulation at 50 Hz is performed to confirm that the cannula tip is adjacent to the target sensory nerve (the patient should report a reproducible, non-painful buzzing or tingling sensation in the knee at less than 0.5 V). Motor stimulation at 2 Hz is then performed to confirm the absence of motor nerve proximity (no visible or palpable muscle contraction in the lower leg or foot). Once concordant sensory stimulation is confirmed and motor stimulation is negative, 0.5-1 mL of 2% lidocaine is injected through the cannula to anesthetize the target nerve, and the radiofrequency generator is activated to heat the active tip to 80 degrees Celsius for 90 seconds. The process is repeated at each of the three target sites.
Genicular nerve RFA is an outpatient procedure requiring approximately 30-45 minutes. The patient is awake throughout and may receive mild oral or intravenous sedation if needed for anxiety. The most uncomfortable portion of the procedure is typically the sensory stimulation phase, during which the patient may experience a brief, intense buzzing or tapping sensation in the knee — this is expected and confirms that the cannula is optimally positioned. Some patients also experience transient burning discomfort during the active heating phase, which resolves immediately when the generator cycle is complete.
Following the procedure, patients may experience mild soreness and swelling at the cannula insertion sites for 2-4 days. There is typically a period of 1-3 weeks before the full analgesic effect is appreciated, as it takes time for the Wallerian degeneration of the ablated nerve fibers to complete and for the surrounding inflammatory response — triggered by the thermal injury itself — to subside. Patients are instructed to apply ice to the knee as needed for comfort and to avoid heavy lifting, prolonged standing, and strenuous lower extremity activity for one week.
In the first week following the procedure, patients may continue to experience their baseline level of knee pain or even a slight increase in discomfort due to the acute thermal injury. Anti-inflammatory medications are generally avoided during this period as they could interfere with the lesion size and durability. Patients are encouraged to resume light activities of daily living as tolerated. By weeks 2-4, the therapeutic effect of the ablation becomes apparent as the nerve degeneration completes and the inflammatory phase resolves. Patients typically report progressive improvement in pain, stiffness, and function during this period, with the maximum therapeutic effect usually achieved by 4-8 weeks post-procedure.
Physical therapy is a critical component of the recovery program. Once pain relief is established, targeted quadriceps and hamstring strengthening, gait training, and proprioceptive re-education help patients maximize the functional benefits of reduced pain. Most patients are able to significantly increase their activity tolerance, walking distance, and participation in recreational activities compared with their pre-procedure baseline.
Patient selection is the single most important determinant of a successful outcome from genicular nerve RFA. The ideal candidate has moderate to severe knee pain that is predominantly nociceptive in nature (deep, aching, worsened by weight-bearing and improved by rest), has clear radiographic evidence of osteoarthritis (Kellgren-Lawrence grade II-IV), and has achieved 50% or greater pain reduction following diagnostic genicular nerve blockade. Patients with primarily inflammatory arthritis (rheumatoid, psoriatic), severe fixed flexion deformity, ligamentous instability requiring surgical reconstruction, or pain that is predominantly mechanical in nature (locking, catching, giving way due to loose bodies or meniscal pathology) are less likely to benefit and should be evaluated for alternative treatments.
Contraindications include active infection at the procedure site, coagulopathy or therapeutic anticoagulation that cannot be safely interrupted (this is critically important given the periosteal target and the risk of bleeding with multiple cannula placements), known allergy to local anesthetics, preexisting neuromuscular disease affecting the lower extremity, and peripheral neuropathy that would compromise the accuracy of sensory stimulation testing.
